Ruby faces new challenges as a blistering heatwave hits Kerala. Lydia decides Ruby is ready for her first solo night shift, but when a storm hits Barco, Ruby faces an agonising test that stretches her medical skills to the limit.

Lydia discovers that a former patient is in the care of a herbal doctor and is determined to win back her patient's trust. Meanwhile, Ruby and Gabriel treat a patient with a mystery condition - and Gabriel reveals something about his past.

Gabriel joins Ruby on an outreach programme to a tea plantation but he discovers that Ruby hasn't been open about her motive for the trip. Lydia attempts to help a patient who she suspects may be a victim of domestic abuse.

Ruby is still reeling from her discoveries at the tea plantation and must decide if she can forgive past mistakes. Mari goes head to head with Lydia when a surrogate mother gives birth in the hospital.

A building collapses in Barco and the team are stretched to the limit. Ruby’s objectivity is put to the test when she discovers a secret about one of her patients and Ram cares for a patient whose husband is missing.

Ruby and Gabriel intervene in a dispute between two fishermen and Lydia faces a difficult dilemma when her former mentor asks for her help. The team come together for Ram and Mala’s wedding and Ruby and Gabriel have a heart to heart.
